HOSPITAL NOTICE OF PRIVACY PRACTICES

THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU MAY ACCESS THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.

This notice tells you the ways we may use and disclose medical information about you. Not every use or disclosure in a category will be listed but all of the ways we are permitted to use and disclose information will fall within one of the categories. We reserve the right to change this notice. We reserve the right to make the revised notice effective for medical information we already have about you as well as any information we receive in the future. We will post a copy of the current notice in plain view. Each time we revise our notice, we will post it and will also make it available to you upon request.

We also describe your rights and certain obligations we have regarding the use and disclosure of medical information. We are required by law to:

  • Make sure that medical information that identifies you is kept confidential;

FOR TREATMENT: We may use medical information about you to provide medical treatment or services. We may disclose medical information about you to doctors, nurses, or other personnel in our organization who are involved in taking care of you. For example, we may need to tell a therapist about your condition in order to coordinate your therapy. We also may disclose medical information about you to health care providers outside our organization who are involved in your treatment, such as consulting physicians or therapists. Also, we may share medical information about you in order to coordinate different things you may need such as prescriptions, lab work, and x-rays.

FOR PAYMENT: We may use and disclose medical information about you so that the services you receive from us or other providers may be billed and payment may be collected from you, an insurance company or a third party. For example, we may need to give your medical insurance plan information about treatment you received, so it will pay us or reimburse you for the treatment, or to obtain prior approval or determine whether your plan will cover the treatment.

FOR HEALTH CARE OPERATIONS: We may use and disclose medical information about you for our operations and to make sure that you receive quality care. For example, we may use medical information to review our treatment and services and to evaluate the performance of our staff in caring for you. Some of these reviews may be conducted by independent physicians who are members of the medical staff but who are not our employees. We may disclose medical information to “business associates” who provide contracted services such as accounting, legal representation, claims processing, accreditation, and consulting. If we do disclose medical information to a business associate, we will do so subject to a contract that provides that the information will be kept confidential. We may also combine the medical information we have with medical information from other similar organizations to compare how we are doing and see where we can make improvements in the care and services we offer. We may remove information that identifies you from this set of medical information so others may use it to study health care and health care delivery without learning who you are.

CENTRAL OPERATIONS: This hospital is a Blue Sky Therapy, Inc. (Blue Sky) facility. Blue Sky is a covered entity for purposes of complying with the federal Privacy Standards. Most of our payment and health care operations are performed centrally by Blue Sky, and certain medical information about you may be shared with Blue Sky for those purposes. For instance, we may share information about you for billing, quality assurance, and compliance purposes or in dealing with our employees. We do not share identifiable information about you with other Blue Sky facilities unless it is in connection with your treatment (for example, if you are transferred to one of our other hospitals), payment, or our health care operations (such as looking at trends among Blue Sky facilities).

OTHER USES OF MEDICAL INFORMATION

For uses and disclosures beyond treatment, payment, and operations purposes, we are required to have your written authorization, unless the disclosure falls within one of the exceptions described below. All disclosures of psychotherapy notes require your written authorization, with certain very limited exceptions such as disclosures necessary to protect your safety or the safety of others or as otherwise required by law. If you give us authorization to use or disclose medical information about you, you may revoke that authorization, in writing, at any time. If you revoke your authorization, we will no longer use or disclose medical information about you for the reasons covered by your written authorization. You understand that we are unable to take back any disclosures we have already made with your permission.

  • We may use and disclose medical information to contact you to remind you of an appointment for treatment.
  • We may use and disclose medical information to tell you about or recommend treatment options or alternatives or other health-related benefits or services that may be of interest to you.
  • Unless you object, we may include certain limited information about you in our directory while you are a patient here. This information may include your name, location, your general condition (e.g., fair, stable, etc.) and your religious affiliation. The directory information, except for your religious affiliation, may be released to people who ask for you by name. Your religious affiliation may be given to a member of the clergy, such as a priest or rabbi, even if they don’t ask for you by name. This is so your family, friends and clergy can visit you while you are a patient with us and generally know how you are doing. You have the right to ask us to restrict some or all of the information in the directory and to whom it is given.
  • If you do not object, we may release medical information about you to a friend or family member who is involved in your medical care. In addition, we may disclose medical information about you to an entity assisting in a disaster relief effort so that your family can be notified about your condition, status and location.
  • We may use or disclose your information for fundraising activities, but such information will be limited to your demographic information such as your gender and your address, and the dates you received services at our hospital. You will have an opportunity to refuse to receive these communications.
  • Under certain circumstances, we may use and disclose medical information about you for research purposes. We will almost always ask for your specific permission if the researcher will have access to information that reveals who you are, or will be involved in your care.
  • We will disclose medical information about you when required to do so by federal, state or local law.
  • We may use and disclose medical information about you when necessary to prevent a serious threat to your health and safety or the health and safety of the public or another
  • person. Any disclosure, however, would only be to someone able to help prevent the threat.
  • If you are an organ donor, we may release medical information to organizations that handle organ procurement or organ, eye or tissue transplantation or to an organ donation bank, as necessary to facilitate donation and transplantation.
  • If you are a member of the armed forces, we may release medical information about you as required by military command authorities.
  • We may release medical information about you to your employer for workers compensation or similar programs as required by law. When that occurs, we will give you notice about the disclosure.
  • We may disclose medical information about you for public health activities (such as reports of communicable diseases, births and deaths, child abuse or neglect, reactions to medications or problems with products), and if required by law, to notify a person who may have been exposed to a disease or may be at risk for contracting or spreading a disease, or to notify the appropriate government authority if we believe a patient has been the victim of abuse, neglect or domestic violence.
  • We may disclose medical information to a health oversight agency such as Medicare for activities such as audits and investigations that are necessary for the government to monitor the health care system, government programs, and compliance with civil rights laws.
  • If you are involved in a lawsuit or dispute, we may disclose medical information about you in response to a court or administrative order. We may also disclose medical information about you in response to a subpoena, discovery request, or other lawful process by someone else involved in the dispute, but only if a reasonable effort has been made to tell you about the request or to obtain an order protecting the information requested.
  • We may release medical information if asked to do so by a law enforcement official in response to a court order, subpoena, summons or similar process; to identify or locate a suspect, fugitive, material witness, or missing person; about the victim of a crime if, under certain limited circumstances, we are unable to obtain the person’s agreement; about a death we believe may be the result of criminal conduct; about criminal conduct at this organization; and in emergency circumstances to report a crime, the location of the crime or victims, or the identity, description or location of the person who committed the crime.
  • We may release medical information to a coroner or medical examiner. This may be necessary to identify a deceased person or determine the cause of death. We may also release medical information about patients to funeral directors as necessary to carry out their duties.
  • We may release medical information about you to authorized federal officials for intelligence, counterintelligence, and other national security activities authorized by law.
  • We may disclose medical information about you to authorized federal officials so they may provide protection to the President of the United States, other authorized persons or foreign heads of state or conduct special investigations.
  • If you are an inmate of a correctional institution or under the custody of a law enforcement official, we may release medical information about you to the correctional institution or law enforcement official if necessary.

RIGHTS REGARDING MEDICAL INFORMATION ABOUT YOU

You have the right to:

  • Request a restriction or limitation on the medical information we use or disclose about you for treatment, payment or health care operations, or to someone who is involved in your care. For example, you could ask that we not use or disclose information about a procedure you had.
  • Request that we communicate with you about medical matters in a certain way or at a certain location. For example, you can ask that we only contact you at work or by mail.
  • Inspect and receive a copy of the medical information that may be used to make decisions about you. This generally includes your medical and billing records, but does not include psychotherapy notes.
  • Ask us to amend the medical information we have about you if you feel it is incorrect or incomplete.
  • Request a list of the disclosures we made of medical information about you, except for treatment, billing and health care operations, or as a result of your written or verbal authorization (such as for listing in the facility directory).
  • Request a paper copy of this Notice.

To exercise these rights, make the request in writing. Ask our facility’s Privacy Official for the proper form. We have the right to deny your request in certain limited circumstances.

QUESTIONS AND COMPLAINTS

If you believe your privacy rights have been violated you may file a complaint with us by contacting our facility’s Privacy Official at the number for our facility below. Portfolio of Services

Blue Sky Therapy Inc. provides professional therapy services for Physical Therapy, Occupational Therapy and Speech Therapy in the home setting. Our therapists provide services within the scope of a home health agency including, but not limited to Therapy Evaluations, Follow-ups, Start of Care Evaluations, Agency Discharges and Competency Evaluations on other therapist.

We offer three types of documentation submission: hand written, computerized or electronic submission.

Hand written notes are for therapists who exhibit excellent legible hand writing and who are not computer savvy. The level of proficiency of the therapist who prefers this method must be extraordinary and consistent. These notes need to be mailed/dropped off to the office consistently per Blue Sky Therapy policy for this to be an option.

Computerized notes are similar to the electronic submission forms only in look, but not function. Set them side by side, they are nearly identical. Yet, the disadvantage with these is you still have to submit by mail or hand deliver to the office per Blue Sky Therapy policy. Neither the hand written notes or the computerized notes are as practical as our electronic submission system.

Our electronic submission system is state of the art, compliant with all HIPPA/Medicare regulations and delivers with the speed of the World Wide Web. It is highly secure encryption technology, offering real time on-line access to clinical documentation. It is as easy to operate and navigate as your own personal email. Each form takes less time to complete than hand written.

This service is provided to you in cooperation through our partner Home Healthcare SOS, Inc info@homehealthcaresos.com Our system is a heavily modified version of the basic system offered by this company. It is comprised of our copy written forms with the intellectual rights for the modified function of our system belonging to Blue Sky Therapy Inc.

Skilled Therapy Services (STS) Division

Our STS division serves your community by providing and managing physical, occupational and speech therapy services. Our programs are designed to provide quality professional services with superior patient outcomes. We are known for high levels of patient satisfaction, quantitative improvements in their function and detailed documentation at a low price.

Our outpatient programs provide aggressive, hands-on therapy to patients experiencing work- and sports-related injuries, back pain and dysfunction, joint replacement, multiple trauma, post-surgical conditions and other impairments.

We offer a very friendly, knowledgeable, dependable staff who skillfully keep the emphasis on patient care, rather than documentation. What differentiates us from other companies is that our office staff is composed of actual licensed therapy and nursing professionals who have a vast background and experience in home health. Business Models

Our flexible business models offer an array of collaborative opportunities for clients' and therapists evolving therapy needs.

Standard Contracts

Through standard contracts, our rates may be higher. This is due to the often irregularity of referrals and the cost associated for us to maintain the current credentials of the therapists we have assigned to you. To maintain a standard contract with our company the agency must maintain a minimum of two patients on our case load, on average at all times.

Exclusivity Contracts

Through exclusivity contracts, we are able to form a stronger clinical partnership. By guaranteeing all the patient referrals you have for a particular area, in enables us to drive down costs. We are a, ‘Volume Based,’ company and our priority for patient care is second to none. By signing an exclusivity contract with us, it will bring long-term stability to your agency without the usual difficulties and excessive cost associated in dealing with multiple subcontracting companies.

Quality of Care with Exclusivity Contracts

Because we place such a high value on quality of patient care and satisfaction, there may be times when we cannot accept patient referrals because of our concern it maybe compromised. Our staff will notify your agency immediately if this is the case so that you can quickly reassign the patient. This is part of our core value system.

Joint Venture Partnerships with Therapists

Through joint venture partnerships, we allow healthcare providers to take advantage of our clinical, organizational and management expertise to create innovative programs for their market, without sharing the risks. Our joint venture relationships typically result in a more relaxed and comfortable life for the therapists. It enables them to spend more time with family.

We allow independent therapists to take advantage of our clinical, organizational and management expertise by forming a joint partnership. This partnership enables the clinician to use our office as theirs. Our office staff will field their calls, accept patients, create a treatment schedule and email it to you the next day. All the therapists have to do is go see their patients and complete the paperwork. You will have access to our state of the art online submission system. On-Call Service

This is a service we provide with both standard and exclusivity contracts. This is a special service reserved for emergency situations and those patients who simply cannot wait for the next working business day. This service is for after regular business hours and on holidays. Cost for this service is, at minimum, double the normal rate.

Blue Sky Therapy was started in 2004, as a DBA for a young physical therapist. It was established out of necessity and the request of a single home health agency for management and billing purposes. Since then, the company has incorporated and grown considerably under the guidance of its therapist owned and run office.

The core business of Blue Sky Therapy Inc. was the provision of physical therapy services provided in the home. A company specializing in all rehabilitation made good sense and would bring a better complement of services to their clients. Since that time, we have been actively diversifying our business operations giving us a balanced collection of healthcare services which now include Occupational and Speech therapy.

Why are you in the home health care industry?
I can’t think of a more rewarding area of healthcare. I have always enjoyed working within a clinically focused environment. I've worked at both healthcare and non-healthcare companies and there is a difference in the level of personal satisfaction that I get from knowing that the company I founded and work for makes a true difference in someone’s life. The amazing work our therapists do with their patients is something that I admire and respect. Going to work every day knowing that I play a small part in the goal to improve someone’s life is very satisfying.

Express your passion for the elderly?
The seniors in my life, grandparents and friends, have been special in so many ways. I've been lucky to have had a chance to know them and learn about dignity, respect and compassion.

What is your most treasured moment in long term care?
To say that there is only a single moment would be a huge understatement. Overall, watching our patients become functionally normal and safe in their home after entering the hospital or a long term care facility. Teaching them good physical habits that will help improve their lives. Giving them hope where there was only despair. It gives me a great sense of satisfaction and pride that this company, through all of its staff and therapists, is able to extend such a high level of caring and compassion to so many.

Brief description of your area of responsibility:
Managing and working in our rehab business in California. I am responsible for ensuring that our business remains healthy and vibrant so that we can continue to offer the highest quality of care available. I also maintain the core values system this company was founded on and ensure that each of our staff and therapists understand and exhibit them.

Any other information or fun facts you may want to include? Family, pets, famous quotes?:
I am a native Nebraskan and a fifth generation born on my family’s farm which is still in operation today. It raises corn, wheat, potatoes, soybeans, sunflowers, cane, alfalfa and pinto beans. My father taught me you must diversify and work hard for everything you do. I have brought this philosophy with me and apply it on a daily basis. When I am not at work, I am either at the gym, working out in my home gym or running with my two boxer dogs.

When problem patients are making your life miserable, Jon has intervened and if the issues are serious he will go to see the patient, and resolve the problem if possible. If not possible he will discharge the patient, or at least get the problem patient removed from the employees list of patients.

Jon pays well and he will work with therapists that have other jobs, but need additional income. Blue Sky is a pleasant place to work, and I urge quality therapists to give him a call.”

“It is never enough to do just the minimum in our jobs. Today, I had the opportunity to see the importance of understanding the fact where body physiology is concerned. Physical Therapists are not there just to work muscles or teach gait training or safety precautions. They need to remember that without the total body working properly, a patient might experience a physiological break-down. Jon Polly reminded me of that very kind of physiology when he took the time to do what he has been trained to do, but which is not always the most cost-effective. Blue Sky Therapy Inc. provides professional therapy services for Physical Therapy, Occupational Therapy and Speech Therapy in the home setting. Our therapists provide services within the scope of a home health agency including, but not limited to Therapy Evaluations, Follow-ups, Start of Care Evaluations, Agency Discharges and Competency Evaluations on other therapist.

We offer three types of documentation submission: hand written, computerized or electronic submission.

Hand written notes are for therapists who exhibit excellent legible hand writing and who are not computer savvy. The level of proficiency of the therapist who prefers this method must be extraordinary and consistent. These notes need to be mailed/dropped off to the office consistently per Blue Sky Therapy policy for this to be an option.

Computerized notes are similar to the electronic submission forms only in look, but not function. Set them side by side, they are nearly identical. Yet, the disadvantage with these is you still have to submit by mail or hand deliver to the office per Blue Sky Therapy policy. Neither the hand written notes or the computerized notes are as practical as our electronic submission system.

Our electronic submission system is state of the art, compliant with all HIPPA/Medicare regulations and delivers with the speed of the World Wide Web. It is highly secure encryption technology, offering real time on-line access to clinical documentation. It is as easy to operate and navigate as your own personal email. Each form takes less time to complete than hand written.

This service is provided to you in cooperation through our partner Home Healthcare SOS, Inc info@homehealthcaresos.com Our system is a heavily modified version of the basic system offered by this company. It is comprised of our copy written forms with the intellectual rights for the modified function of our system belonging to Blue Sky Therapy Inc.

“It is never enough to do just the minimum in our jobs. Today, I had the opportunity to see the importance of understanding the fact where body physiology is concerned. Physical Therapists are not there just to work muscles or teach gait training or safety precautions. Our flexible business models offer an array of collaborative opportunities for clients' and therapists evolving therapy needs.

Standard Contracts

Through standard contracts, our rates may be higher. This is due to the often irregularity of referrals and the cost associated for us to maintain the current credentials of the therapists we have assigned to you. To maintain a standard contract with our company the agency must maintain a minimum of two patients on our case load, on average at all times.

Exclusivity Contracts

Through exclusivity contracts, we are able to form a stronger clinical partnership. By guaranteeing all the patient referrals you have for a particular area, in enables us to drive down costs. We are a, ‘Volume Based,’ company and our priority for patient care is second to none. By signing an exclusivity contract with us, it will bring long-term stability to your agency without the usual difficulties and excessive cost associated in dealing with multiple subcontracting companies.

Quality of Care with Exclusivity Contracts

Because we place such a high value on quality of patient care and satisfaction, there may be times when we cannot accept patient referrals because of our concern it maybe compromised. Our staff will notify your agency immediately if this is the case so that you can quickly reassign the patient. This is part of our core value system.

Joint Venture Partnerships with Therapists

Through joint venture partnerships, we allow healthcare providers to take advantage of our clinical, organizational and management expertise to create innovative programs for their market, without sharing the risks. Our joint venture relationships typically result in a more relaxed and comfortable life for the therapists. It enables them to spend more time with family.

We allow independent therapists to take advantage of our clinical, organizational and management expertise by forming a joint partnership. This partnership enables the clinician to use our office as theirs. Our office staff will field their calls, accept patients, create a treatment schedule and email it to you the next day. All the therapists have to do is go see their patients and complete the paperwork. You will have access to our state of the art online submission system. On-Call Service

This is a service we provide with both standard and exclusivity contracts. This is a special service reserved for emergency situations and those patients who simply cannot wait for the next working business day. This service is for after regular business hours and on holidays. Cost for this service is, at minimum, double the normal rate.

Common Questions

Do I refer patients directly to Blue Sky Therapy?
You can refer private pay and Medicare part B directly to us. A home health agency is required for all other patients to handle all other insurance coverage requirements, including Medicare Part A and Medi-Cal. We do not take any private insurance coverage patients. This is because, we are considered a non-traditional home based service entity with no accrediting body to enable us to become part of any of the commercial insurance networks. We do work for several agencies that do accept private insurance and would be glad to handle all referrals you may have in this way.


What if I want Blue Sky Therapy to see my patients but don't what to change Home Health Agencies?
You can request for the Blue Sky Therapy management to obtain a contract with the agency you are comfortable referring patients to. Once a contract is established, you can also personally request that Blue Sky Therapy is to be the primary provider of skilled home based services to your patients. In this way, you know your patients are being taken care of exactly the way you want them to be.


Is Blue Sky Therapy perfect?
No, we are not perfect. When we make a mistake, we own up to it and take responsibility for it. We then do whatever is necessary to ensure that the issue in question does not reoccur. There are many factors that come into play when it comes to the logistics of providing a skilled therapy service in the privacy of someone's home. Our experience and high level of professionalism give us a solid understanding of this. The few mistakes we do make, we appreciate them, because we use them as a learning experience. This ultimately makes us better at what we do.


Can I see an example of Blue Sky Therapy's Home Exercise Program?
Of course, you can. Simply have one of your staff set up a time for Mr. Jon Polly RPT to come in and meet with you personally. He will show you examples of what we are currently using with other MD's and, you can choose the type and style which best represents your needs and interests.


How much effort on my part does it take to create my own customized Home Exercise Program for patients?
Not much at all. It may take several revisions and meetings with Mr. Jon Polly RPT before we have one that is functional and meets, or exceeds, your requirements. However, all that is required from you is your input on requirements, feedback, and a few moments of your time.


Aren't all Home Health Therapists the same?
No they are not. Just like each doctor has their own way of treating patients, so do therapists. Not all therapists are going to go out of their way to follow your protocol or treat your patients in the manor you prefer. With Blue Sky, you only have to tell us something once. Our therapists are, 'goal oriented,' and aggressive.

Blue Sky Therapy was started in 2004, as a DBA for a young physical therapist. It was established out of necessity and the request of a single home health agency for management and billing purposes. Since then, the company has incorporated and grown considerably under the guidance of its therapist owned and run office.

The core business of Blue Sky Therapy Inc. was the provision of physical therapy services provided in the home. A company specializing in all rehabilitation made good sense and would bring a better complement of services to their clients. Since that time, we have been actively diversifying our business operations giving us a balanced collection of healthcare services which now include Occupational and Speech therapy.

Why are you in the home health care industry?
I can’t think of a more rewarding area of healthcare. I have always enjoyed working within a clinically focused environment. I've worked at both healthcare and non-healthcare companies and there is a difference in the level of personal satisfaction that I get from knowing that the company I founded and work for makes a true difference in someone’s life. The amazing work our therapists do with their patients is something that I admire and respect. Going to work every day knowing that I play a small part in the goal to improve someone’s life is very satisfying.

Express your passion for the elderly?
The seniors in my life, grandparents and friends, have been special in so many ways. I've been lucky to have had a chance to know them and learn about dignity, respect and compassion.

What is your most treasured moment in long term care?
To say that there is only a single moment would be a huge understatement. Overall, watching our patients become functionally normal and safe in their home after entering the hospital or a long term care facility. Teaching them good physical habits that will help improve their lives. Giving them hope where there was only despair. It gives me a great sense of satisfaction and pride that this company, through all of its staff and therapists, is able to extend such a high level of caring and compassion to so many.

Brief description of your area of responsibility:
Managing and working in our rehab business in California. I am responsible for ensuring that our business remains healthy and vibrant so that we can continue to offer the highest quality of care available. I also maintain the core values system this company was founded on and ensure that each of our staff and therapists understand and exhibit them.

Any other information or fun facts you may want to include? Family, pets, famous quotes?:
I am a native Nebraskan and a fifth generation born on my family’s farm which is still in operation today. It raises corn, wheat, potatoes, soybeans, sunflowers, cane, alfalfa and pinto beans. My father taught me you must diversify and work hard for everything you do. I have brought this philosophy with me and apply it on a daily basis. When I am not at work, I am either at the gym, working out in my home gym or running with my two boxer dogs.

What is our Service?

We provide onsite or in home rehabilitation program for persons throughout southern California. Our program brings skilled therapy services right where they are most needed—we provide rehabilitation services and care coordination onsite at home.

By delivering our services in the patient's home environment - the results are clear: increased safety, higher compliance, less relapse through attainment/understanding of good physical habits, better attendance, more complete participation in therapy and quicker recovery if program followed consistently/daily basis. For some of our patients, home therapy is likely the only realistic option - especially when travel, fatigue or other barriers prevent attendance at traditional outpatient clinic-based therapy.

Many of our patients are referred by local physicians who are looking for a way for their mobility restricted patients to benefit from therapy without having to travel beyond their abilities. Other patients are referred by community agencies that serve the senior and low mobility community. Additionally, many of our patients are referred to us from Home Health Agencies or traditional outpatient clinics (when patients struggle to attend or arrive too fatigued to participate completely).

Who is Eligible to Receive Home Health Care Services under the Medicare Benefit?

To qualify for home health services a patient must:

  • Be homebound—requiring assistance to leave home;
  • Need intermittent care or instruction;
  • Require the services of a skilled Healthcare professional that will coordinate and provide medical treatments or special procedures prescribed by the patient’s physician; and
  • Must be under the care of a physician who is willing to provide the Agency with orders.

Who Pays for These Services?

Approved services are covered 100% by Medicare and Medi-Cal. Patients may be covered under insurance policies, or may pay privately for home health services. A Blue Sky Therapy can provide coverage for you under Medicare Part B or private pay. A home health agency handles all other insurance coverage requirements, including Medicare Part A, Medi-Cal and processes all claims.

Who Is This Program For?

It is designed for the general community—especially in areas with a high proportion of senior citizens likely to benefit from rehabilitation services but who may not have ready access to those services. This program is primarily intended for patients who are truly homebound, as defined by Medicare, who also require skilled therapy services.

Patients who no longer qualify for service under Medicare Part A, countenance of therapy under Medicare Part B, private pay or Out-Patient Services are options.

Our typical patient is someone who is not truly home bound but may not be as mobile as they wish or who may not feel comfortable traveling to a clinic for needed therapy services. Some of our patients simply prefer to be seen in the comfort of their homes.

What Are the Features of In Home Therapy?

Onsite and in home rehabilitation services by fully qualified and licensed by the state of California physical therapists, occupational therapists and speech therapists in coordination with you and the patient’s own local physician.

What to expect with Home Care Visits:

A therapist or staff member will call you to set up a time for the initial evaluation. Evaluations are required before any treatment can be given for the patient’s own safety and to follow professional/ethical guidelines. A ‘window,’ of time will be required to allow the therapist to arrive at your home safely and to enable the therapist to accommodate the other patients, on the schedule, in a courteous fashion.

On the day of the evaluation, you can fully expect the therapist to arrive within the window of time requested. For example, “Mr. Polly will be at your home between 12-2 pm.” If the therapist will not arrive within the time, you will be notified.

A clean, courteous, punctual, professional therapist will arrive at your place of residence. The evaluation begins when the therapist arrives outside of your home. Everything is taken into consideration when it comes to home therapy. From the curb outside of your home, to the slopping drive way or the 20 steps leading to your door; the logistical topography of your surroundings will be considered before a personal home exercise program will be given to you.

The therapist will ask you a number of questions about the last several months of your life leading up to this moment. There is always a reason (s) why a therapist has come to see you and it is their job to discern exactly what they are and what you need help with the most. Some information is always provided to the therapist before they see you, however it is often incomplete and not always appropriate for what the therapist is really at your home for.

Purpose of the Therapist: Safety, Stability, Alleviating fears, Education, prevention and creating some good habits that, over time, can lead to increased independence.

A customized home exercise program will be given to you on the day of evaluation. You will then be assigned a therapist who will see you 1-3 times a week depending on your need and diagnosis. You will be notified by phone, the name of the follow up therapist.

Our overall goal is to teach you good physical habits that you will carry on the rest of your life. Your body is the vessel you travel through life in. It must be taken care of to ensure an enjoyable cruise.

How Does It Work?

Once you identify a person who has the need for rehabilitation services but is not qualified or appropriate for home health or clinic based therapy, simply call us at 818-831-2400 or fax it to us at 818-831-2411 with a referral. A physician referral is helpful but not required (We will be happy to contact the patient’s physician for the referral to therapy). As soon as we have the information, we will begin.

If the patient is eligible for home health agency services, we can give you a list of home health agencies with whom you can proceed. Our list of preferred home health agencies is based on our past experience with their staff members and their overall professionalism!

Is this Program Covered by Health Insurance?

For most senior citizen patients, the rehabilitation services are covered under Medicare Part B (not part of the Home Health Agency Prospective Payment System) with only a small co-payment by co-insurance. We always verify insurance coverage prior to initiating services.

For most of our patients, who have Medicare Part A, and qualify under the definition of homebound, our Services are covered through a home health agency!

We do not take any private insurance coverage patients due to the fact that our particular company is considered a non-traditional home based service entity with there being no accrediting body to enable us to become part of any of the commercial insurance networks.
